How to Sign Up for Athletics Schoology Group

Parents/Guardians and Student-athletes,
All students can participate in interscholastic sports at Franklin Middle School.

Student-Athletes are required to:

● submit a Physical Evaluation form before they may begin tryouts/practice,
signed/stamped by an authorized medical examiner.
● complete the online registration that provides important health, safety, and emergency
contact information.
● be academically eligible. (Based on the previous quarter’s report card, students must
have a 2.0 GPA average with no more than one E.)
● demonstrate good sportsmanship and positive citizenship, both on and off the playing
field and in the community.
● follow all COVID-19 related protocols required by BCPS.

Physical Evaluation Forms:

Physicals are now valid for 14 months from the physical exam date.
Ex: If your new physical exam date is 
2/1/21, it expires on 4/1/22. It is strongly suggested that all athletes get a new physical after June 1,
2021. Any physical after that date will be good for all three sport seasons of the 2021-22 school year.

Follow these steps to submit your athletic physical:

1. Print out all pages of the UPDATED 2021-2022 PHYSICAL EVALUATION FORM.
a. Click English or Spanish for the version of the form you need.
b. *Note: this is a new form that now includes an additional COVID-19 page that MUST be completed.
2. Complete sections of the form for the parent/participant before going to the doctor.
3. Have your doctor/authorized medical examiner complete all required sections of the form. They must also sign and stamp the form. If you have had an exam within the calendar year, you may be able to forward these forms to the doctor to complete.
4. Once completed by the Authorized Medical Examiner, submit the “Physical,” “History,” and “COVID-19” pages in order to participate.
a. Upload to your online registration, and ADD the DATE OF YOUR PHYSICAL.

Please use this link to the Franklin Middle School Form REleaf website to register for athletics!

Log in to your account, or if this is your first visit - create an account. Save your username and password 
for future access to this registration portal.
To complete the registration process you will need to provide contact information for the athlete, parents/guardians, and emergency contacts, as well as Health insurance and Doctor information.
You will also choose which sport you are signing up for. You are allowed to register for ONE sport PER SEASON. Each season’s sports will open up close to the season start date, so some sports may look
locked/closed, however they will open closer to the start of that sport season, so don’t worry!

Here is the breakdown:

FALL - register Aug 23  WINTER - register Oct 1 SPRING - register Mar 1
Badminton Girls Basketball Tennis
Cross Country Boys Basketball Track & Field
Ex: A student can register for Badminton in the Fall, Basketball in the Winter, and Track & Field in the Spring. Plus, here at FMS, this student could also still participate in other intramural activities such as our DragonBall Basketball tournament and also attend Archery practices and events!

Academic Eligibility:

BCPS will be modifying its student athlete academic eligibility policy (Rule 6702) to allow students the opportunity to participate in the BCPS Interscholastic Athletic Program for the 2021 FALL athletic season
as long as the student is making satisfactory progress towards graduation. The interpretation of this modification WILL ALLOW ANY STUDENT TO BE ELIGIBLE FOR ATHLETIC PARTICIPATION IF THEY EARNED

Standard eligibility requirements will resume for students at the start of the second quarter. This policy 
states that a student-athlete must have at least a 2.0 average GPA and no more than one failing grade

BCPS COVID Protocol and Reporting:

There are no spectators allowed at practice. If a parent/guardian needs to remain on campus during 
practice, they must stay in their vehicle and are not able to access the building or fields.

More information will be provided on the current COVID protocol for athletes at practice, and both 
athletes and spectators at future events, such as our culminating cross country/track & field meets, our badminton/tennis tournaments, and our home/away basketball games.
